Revitalize Massage Therapy

1. Choose between a 60, 90 or 120 minute massage below. Booking a longer massage will give more time to address areas of concern and incorporate several techniques to achieve the best result. A card is required to request an appointment but will not be charged. For 3+ people, please inquire.

2. After requesting an appointment, you will immediately be prompted to complete a new client intake and consent form that must be filled out prior to your massage. This is solely for compliance with regulations. Your privacy is protected.

3. Payment is due after the massage is completed. Acceptable forms of payment are credit, debit, venmo. No cash or check. Group events require a 50% deposit.

4. For cancellations, please give at least a 24 hours’ notice if you wish to cancel or reschedule your appointment. If you cancel under 24 hours, you will be charged 50% of the scheduled service. If the appointment is a no call/no show, you will be charged the full amount.
